At Strategic Therapeutic Solutions, we understand that healing continues beyond the therapy room. We welcome mental health professionals who enjoy meeting people in their everyday environments and helping them build the skills, connections, and confidence to live more independently.
You will join STS’s skilled and compassionate interdisciplinary treatment team, working closely with licensed clinicians, psychiatric providers, peer specialists, and case managers. Together, we coordinate care throughout the client’s healing journey. Your community work helps connect treatment goals with daily routines, essential resources, and meaningful participation at home, work, and in the community.

- Work with clients in approved home, office, and community settings to strengthen coping, daily living, communication, and problem solving skills.
- Connect clients with resources for healthcare, housing, food, benefits, employment, and other needs; follow up on access barriers.
- Coordinate with clinicians, psychiatric providers, peers, and authorized supports to reinforce agreed treatment goals.
- Monitor functioning, engagement, and progress; communicate changes and safety concerns promptly to the clinical team.
- Complete timely EHR records and participate in supervision, competency development, case reviews, and transition planning.
